A questions worksheet gives non-urgent uncertainties a place to wait for the next conversation. Use direct communication for anything that needs an immediate response during care.
Capture the question while it is clear
Write the situation that prompted the question, not just a vague reminder such as “ask about bag.” A useful entry might say that two activity bags are in the cupboard and the caregiver needs to know which one goes on Friday. Include when the answer will be needed so a small question does not remain unresolved until the last minute.
Keep questions separate from complaints or instructions
The worksheet is a place to clarify, not a scorecard of mistakes. Use neutral descriptions and leave space for both people’s understanding. A parent may discover that an instruction was never shared, while a caregiver may offer a simpler approach. Discuss a concern directly when it needs a conversation rather than expecting a written note to carry the whole exchange.
Move the answer to the right place
Once a question is answered, record the agreement and decide whether it belongs in the routine, weekly plan, or household reference. Otherwise, the useful answer may disappear into a notebook that nobody opens again. Mark the question resolved after the update is made. Revisit the open list together at a time that works for the people involved.
