Coordinate the stretch between school and home with a printable plan for collection, belongings, activities, and the next adult taking over.

An after-school plan connects collection to the rest of the afternoon. Name the adult responsible for each change of place, and keep the sequence easy to follow when plans are busy.
Begin at the school finish time
Write the dismissal time and collector, then add the destination immediately afterward. If an activity starts later, show what happens in the gap rather than leaving a blank stretch that two adults interpret differently. Check the school’s collection arrangements separately. This worksheet organizes the family plan and does not change the school’s permissions or procedures for releasing a child.
List only what travels today
The belongings section is for today’s needs: an activity bag, a library book, or clothing to bring back from a previous visit. Avoid copying a permanent packing list into every afternoon. A short list is easier to check at the door. Mark where an item currently is when it may be at school, in a car, or with another caregiver.
Name the last handoff
End with who is taking over, where, and approximately when. If an adult is running late, use the communication method already agreed with the person providing care. Do not silently edit a shared sheet and assume the update has been seen. Review what actually happened afterward only when it changes what the next caregiver needs to know for the evening.
