Keep one current record
Put the dated sheet where the adults providing care can find it. If a parent makes an entry and a nanny takes over afterward, check the latest line together. Do not copy an expected feed from a schedule into the completed log. When a detail is unknown, record that uncertainty rather than filling in a time or amount from memory.
Use the log during the handoff
At the change of caregiver, point out the latest entry and any question that still needs an answer. The person taking over should know who recorded the information and where to ask for clarification.
