A shift handoff sheet focuses on what the incoming caregiver needs at the point of taking over. Use it for the boundary between two people, alongside the fuller daily record.

Start with the current situation

Write the date, handoff time, and names of the outgoing and incoming adults. Then describe what is happening now in a sentence: the child is finishing a puzzle, the bag is packed, or an outing was postponed. This gives the incoming person a starting point without asking them to reconstruct several hours of notes before they can take over.

Highlight the unfinished work

A task that is not complete needs an explicit next owner. Write what remains and whether it is time-sensitive within the family’s plan. Avoid instructions such as “someone should remember” because the recipient cannot tell whether they are being asked to act. Include a question mark where clarification is still needed instead of presenting an uncertain detail as a fact.

Confirm the transfer in person where possible

Read through the short sheet together and allow the incoming caregiver to ask about anything unclear. For an agreed remote handoff, use the communication method both people expect. Keep the detailed day log available for context, but do not duplicate every entry here. After the transfer, store the sheet with that day’s record so responsibility is understandable later.
